- To keep this discounted price: You must complete your next refill check-in within 45 days of your last delivery.
- If you miss the 45-day window: You’ll be charged full price for your refill—$349 for 14.5 mg and 17.2 mg doses.

If you choose not to move forward with an alternate option presented to you, you will receive a refund of your monthly membership fee minus a $45 provider fee.

- Once your order has been approved by your physician, it will be forwarded to the pharmacy for fulfillment. The pharmacy will contact you to collect payment prior to shipping.
